Fill your info in consent form in section 2 i.e Applicant's information, if needed fill your spouse/children information in Related Individual's section. Please do not fill anything in section 1

Once the consent form is filled, upload it via upload button along with your rejection letter and passport. Once files are selected click on submit button to submit the files.

After upload of your documents. Please also email the docs on info@fatehconsultancy.com, so we can finalize the application.
